Andrew is an accomplished and visionary executive with more than 30 years of healthcare leadership experience. He has held executive leadership positions in both rural and urban settings with responsibility for acute, community, and long-term care; mental health and addiction services; health prevention and promotion; Indigenous health; and laboratory services. In addition, he has led strategic planning and managed a broad range of healthcare administrative services.

With a distinctly people-focused approach to leadership, Andrew develops meaningful relationships with stakeholders and communities locally, nationally and internationally that help him identify and recommend quality and safety improvement initiatives suited for Alberta. He brings a sincere passion for and proven commitment to addressing the challenges of health reform and meeting community and provincial needs in a rational and responsible way.

Andrew has held various executive healthcare positions across Canada: notably as Vice President, Community Integration, and Chief Operating Officer for Interior Health in British Columbia. He is a Certified Health Executive and holds a Bachelor of Health Services Administration, a Master of Business Administration, and a Master of Arts in Leadership & Training from Royal Roads University.

Executive Director

Charlene is Executive Director of the HQCA. Charlene has been with the HQCA since 2004, and oversees daily operations, while providing expertise and guidance to ensure the organization meets its legislated mandate and strategic and business plan objectives. She has more than 30 years’ experience in healthcare and has held a variety of leadership roles. Prior to joining the HQCA, Charlene worked within the former Calgary District Hospital Group and Calgary Health Region, Calgary Laboratory Services, and Carewest.

In addition to her formal training as a medical laboratory technologist where she started her career in healthcare, Charlene holds a Master of Business Administration in Executive Management from Royal Roads University.

As Director, Communications, Lisa advises and collaborates within the HQCA to ensure the organization maintains its positive reputation for transparency, credibility, and objectivity. With expertise in strategic communications and planning, media relations, writing and editing, issues management and stakeholder communications, Lisa has oversight for all communication activities within the organization, ensuring its healthcare partners and the public are engaged in the HQCA’s work in a way that is meaningful and appropriate.

Prior to joining the HQCA, Lisa held a variety of communications roles spanning 15 years in several organizations including FortisAlberta and the Alberta Electric System Operator. Lisa holds a Bachelor of Arts in Communications Studies from the University of Calgary and is an Accredited Business Communicator through the International Association of Business Communicators.

Tim is the HQCA’s Senior Lead, Health System Analytics. Tim’s focus includes leading and mentoring staff on the development of surveys, survey methods, and quality measures as well as conducting data analysis and writing summary reports. He has had a critical role developing partnerships and fostering collaborations for HQCA initiatives, working with organizations including Alberta Health, Alberta Health Services, the Canadian Institute for Health Information (CIHI), the Agency for Healthcare Research and Quality (US), the Healthcare Commission (UK), the EuroQol Group (NL), the Commonwealth fund, and the Institute of Health Economics in Alberta. Tim has also been a member of a number of measurement- related working groups both provincially and nationally.

Prior to joining the HQCA in 2005, Tim spent nine years with the former Calgary Health Region, and he also worked with Calgary Health Services and the Alberta Cancer Board.

Dr. Flemons brings a wealth of experience to his role as Medical Director, Health System Improvement. He works with the HQCA to advise on and lead some of the HQCA’s system improvement and measurement projects. Dr. Flemons is a Respirologist at the Foothills Medical Centre and a Professor of Medicine at the University of Calgary’s Cumming School of Medicine. He leads Patient Safety & Quality Education for the W21C (a research and innovation centre in the School of Medicine). He is the former Vice President of Quality, Safety and Health Information for the Calgary Health Region. His leadership led to a number of important initiatives, from the development of four regional patient safety policies (Disclosing, Reporting, Just & Trusting Culture, and Informing) and a regional guideline on the management of serious adverse events, to the implementation of a new electronic safety learning reporting system and the establishment of a Patient / Family Safety Council. Dr. Flemons also led in the creation of a regional patient advocacy program that assisted patients who had experienced adverse events.

Dr. Flemons graduated from the University of Alberta in 1984. After completing his Internal Medicine and Respiratory Medicine training in 1990 he completed a three year AHFMR (Alberta Heritage Foundation for Medical Research) Clinical Research Fellowship. In 2008, he completed a two-year CHSRF sponsored EXTRA (Executive Training for Research Application) fellowship; his project focused on developing and implementing a system wide approach to improving emergency department patient flow.

He has published numerous peer-reviewed research articles, review papers and book chapters, and chaired several international consensus groups in his field of research interest, sleep apnea.

As Director, Collaborative Learning & Education, Margot plays an instrumental role in highlighting the importance of quality and safety with our stakeholders. Margot is part of the core team developing a quality and safety education framework for Alberta, and is a co-coordinator for the Certificate in Patient Safety & Quality Management and the Investigation & Management of Patient Safety Events courses offered in partnership with W21C and the Office of Continuing Medical Education and Professional Development at the University of Calgary.

Margot began her career as a pediatric nurse, where she worked for 17 years in a variety of acute care settings. She has worked in various positions related to quality and patient safety since 2002, with a focus on quality and patient safety education for healthcare providers since 2007.

Margot holds a Master of Education, specializing in workplace and adult learning, as well as a Bachelor of Nursing from the University of Calgary.

As Director, Health System Analytics, Markus oversees the HQCA’s measurement and analytical functions, as well as many of the monitoring and reporting functions such as population-level surveys and health system performance reporting. This involves survey and administrative data analytics, and survey development, methodology, and reporting. Markus plays a key role in the HQCA’s use of objective information to drive actionable improvement that enhances quality healthcare.

Markus joined the HQCA in 2009 following a career with Statistics Canada’s Research Data Centre at the University of Calgary where he was responsible for managing and coordinating daily operations, and conducting population research. He collaborated with other multidisciplinary teams from across Canada to identify and implement strategic and systematic changes to the program.

Markus holds a PhD in sociology with a specialization in health research from the University of Calgary and a Master of Arts from Lakehead University. He has training in both quantitative and qualitative methods.

As Director of the Project Office, Anette oversees and provides project management expertise for various HQCA quality improvement initiatives. Prior to joining the HQCA in 2006, Anette held numerous management positions in the area of acute care, including managing the Rehabilitation Departments at both the Rockyview and Foothills hospital. Her experience also includes working 16 years at several of Calgary’s acute care hospitals as a physiotherapist.

Anette holds undergraduate degrees in psychology and physical therapy as well as a Master of Business Administration from the University of Calgary.

As Director, Health System Improvement & Citizen Engagement, Carmella leads a team of quality improvement and patient safety experts who engage in system-level reviews and other initiatives to drive improvement in Alberta’s healthcare system. She also supports the HQCA’s Patient/Family Safety Advisory Panel on behalf of the organization, ensuring the patient voice is represented in the HQCA’s work.

Prior to joining the HQCA, Carmella was the Administrative Director for the Ward of the 21st Century (W21C) at the University of Calgary. She also held a number of quality and safety leadership roles with the former Calgary Health Region/Alberta Health Services following her original career in healthcare as a respiratory therapist.

Her primary interests are in systems safety and in conducting prospective and retrospective system-level safety reviews. Carmella has completed a number of safety reviews, including leading large-scale, multi-patient reviews and prospective system reviews, has co-authored a book on conducting safety analyses, and continues to teach and mentor others on the topic.

She holds a Respiratory Therapy diploma from SAIT, a Bachelor of Health Sciences from Thompson Rivers University (BC), and a Master of Public Administration from the University of Victoria.

Dr. Wasylenko is Medical Director, Health System Ethics & Policy for the HQCA, where he provides a medical perspective for a number of quality improvement projects, frequently applying a unique ethical lens into the development of HQCA initiatives.

He has held a number of senior leadership positions in healthcare organizations, and in addition to his current role at the HQCA, he is a Clinical Ethicist and chairs the Public Health Ethics Consultative Group for the Public Health Agency of Canada. He is Medical Advisor for the Advance Care Planning/Goals of Care Designation initiative with Alberta Health Services (AHS) and sits on the Seniors Health Strategic Clinical Network of AHS. Dr. Wasylenko is also a Clinical Lecturer in the Division of Palliative Medicine at the University of Calgary, and is a member of the John Dossetor Health Ethics Centre at the University of Alberta.

He began his career as a rural family physician more than 30 years ago and later trained in palliative care, where he has been a palliative care physician for the past 20 years. Dr. Wasylenko assisted in the development of Calgary’s regional integrated palliative care service, initially serving as a Palliative Home Care Consultant.

Jessica is Director, Financial Services with the HQCA, and has accountability for all financial systems and controls within the organization. She is a Chartered Accountant with more than 10 years of work experience in a variety of industries. Prior to joining the HQCA, Jessica spent seven years with an oil and gas service company working in a variety of financial roles and for a number of different projects. Her career also includes time with Ernst & Young providing assurance and advisory services to both public and private sector enterprises. Jessica holds a Bachelor of Commerce from the University of Calgary and she has her Chartered Accountant designation through the Institute of Chartered Accountants of Alberta.

Senior Lead, Project Management Support

As Senior Lead, Project Management Support in the HQCA’s Project Office, Dale plays an important leadership role in developing project management processes and tools for the organization. She also provides her expertise to many quality initiatives conducted by the HQCA, and coordinates business planning processes for the executive team. Dale has been with the HQCA since 2006, bringing an eclectic skill set from her career as a hospital and drug information pharmacist and continuing medical education program designer. Drawing from her dynamic background and experience, Dale has developed a variety of frameworks and education materials for the HQCA.

Dale holds a Master of Education in Distance Education from Athabasca University, a Master of Pharmacology and a Bachelor of Pharmacy from the University of Saskatchewan.
