Physician panel identification

The HQCA is working with family doctors and their primary care networks to help doctors confirm who their ongoing patients are so that they can provide better care. The HQCA will share a small amount of patient information with a family doctor about patients they have seen in the last three years. This will help the doctor sort through all their patient records to identify who their ongoing patients are.
