  • This course can be taken as a stand-alone class or combined with Advanced Investigating and Managing Patient Safety Events course, as the foundation for the Certificate in Investigating and Managing Patient Safety Events
  • Main focus is the Systematic Systems Analysis: A Practical Approach to Patient Safety Reviews (SSA:PSR) methodology
  • A practical, case base 2½ day course
  • Participants will be eligible for continuing education credits from the College of Family Physicians or Royal College

Learning Objectives

At the end of the course, participants will be able to:

  • Explain the difference between a ‘person’ and a ‘system’ model of patient safety
  • Discuss how systems analysis fits into the larger picture of quality and safety management
  • Identify several reasons why how an organization responds to an event is important
  • Create a chronology of events that will assist in understanding some of the contributing factors that led to the occurrence of patient harm
  • Describe various sources of information that can be sought when conducting a systems analysis
  • Use the SAFER Matrix as a method for analysis of information gathered for the purposes of conducting a review of a patient safety event
  • Create recommendations for system improvements that are actionable and measurable
  • Write a summary about the findings of a safety review
