Patients expect safe healthcare. But things can go wrong – even with skilled healthcare providers in environments and spaces designed to be safe.

If something happens and you are harmed, someone should speak with you – and anyone you wish to have with you for support – as soon as possible.

This process is called disclosure. Working with partner organizations and patient and family advisors, the HQCA has developed information for patients who’ve been harmed during healthcare.
