Improving the ability to act on information
We’ve expanded the measures that include Patient Health Numbers (PHNs) for the Confirmed Patient list version. This means you can prioritize care for specific patients, and target those most vulnerable.
Addressing emerging concerns
We’ve added valuable new measures – laboratory utilization rates and sedating medications in older adults – that will help you deliver better evidence-based care while optimizing healthcare resources.
Easier to use – digital improvements
The digital experience gives you tools to dive deeper into patient data, and compare care patterns with peers at a zone, PCN or provincial level. There will also be an option to print a summary of your report.
The digital report is best viewed in the latest version of Google Chrome, Microsoft Edge, or Safari with a screen resolution at or above 1680 x 1050 (though it will work in other web browsers and at lower resolutions). The report has not be optimized for viewing on a tablet or smart phone.
