If you support a physician on quality improvement initiatives, request delegate access to the digital report. The HQCA defines a “delegate” as a person authorized by a physician to view and access the physician’s report.
With the physician’s approval, the HQCA can give a delegate access to the report within two weeks of receiving the request.
Delegate access will be renewed annually.

All of the physician data and access to the data is protected and secure. The reports are confidential and are only provided to the requesting physician and their approved delegates. To start, delegates must request a Primary Healthcare Panel Report. Once physician approval is granted, the HQCA will send delegates their access information within two weeks of receiving the request.
